Parent Information Form
Information was provided during the registration process.

Parent Information Form (signed at registration)
Dropping your child off for practice

For the safety of your child, do not stop in the middle of Stadium Drive to let your player out of the car.  Please pull into one of the parking lots.  We have been advised by the Village of Clemmons that this is also a traffic violation.

Returned Checks

We will gladly accept checks. We use CheckTRACK. If your check is not paid on presentment or is dishonored, you agree to pay the amount allowed by state law ($25.00). We may electronically debit or draft your account for this charge. Also, if your check is returned for insufficient or uncollected funds, your check may be electronically re-presented for payment.

Trash
We need your help in keeping our fields and surrounding areas clean. Team Parents (or a team representative) are asked to oversee this activity for their teams. Please stop by the concession each night and game day to pick up a trash bag. Players, parents, and fans should clean their own area, tie off the trash bags and leave beside the trash barrels. On Saturday’s after the game, please also clean down the sideline before the other team takes the field. Thank you for helping us take care of our fields.

Smoking
Smoking is not allowed at any Pop Warner event.  Additionally, Forsyth County schools have adopted the 100% Tobacco-Free school has a policy that prohibits the use of tobacco products by anyone, including students, staff, and visitors, on school grounds or at school events at all times. This tobacco-free zone includes school premises, school vehicles, and school events, both indoors and outdoors, and both on and off school property.

Pets

Please do not bring your pets to the field.
